You must complete an Employee Accident Report
NEVER ASSUME that someone else will do this for you; YOU need to take control from the beginning. Without this document, it is possible that your accident will go unreported at work. This can directly affect your pay check, as you will be listed as sick and not on compensation. This form can be accessed via the Aeronet; click on “my work” then click on “all forms” or on the new Aveos Portal. |

| After your initial doctor’s visit; you will receive a completed CSST form (# 1936) in triplicate (a copy each for the employer, employee and CSST). As soon as possible, report to the Air Canada clinic and give them the employer’s copy of the CSST paper. As well, YOU must send the appropriate copy to the CSST. Keep your copy in a safe place. | |||||||
| While at the Air Canada clinic, you will be asked to fill out the CSST Worker’s Claim Form (#1939). This form is very important. If your claim is ever contested, for whatever reason, it may come down to what you have written on this form. It is a good idea to bring with you a copy of your accident report, and to consult with a union EH&S representative. | |||||||

Always make sure an EH&S union member of your local EH&S Committee is advised when there is a work accident. They need to be involved in the joint investigation with the employer and will help you with any documentation that you need to fill out.
